The attack succeeded because someone in accounting opened an email attachment. Not because they were careless—because they had never been taught what modern phishing looks like.

That incident revealed a harsh truth: organizations spend fortunes on technology while ignoring the human element. Security awareness training existed, but it was checkbox compliance, not real education.
